What does the userinit key show under this key?

HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows NT\CurrentVersion\Winlogon

Should be just this:

C:\Windows\system32\userinit.exe,

If there is anything appended after the comma, you may have a malware issue.
Have you run a spyware scan recently? The above is not the only location
that these bugs can come from, and many of them can interfere with the
windows shell.
